Confirmation Forms

The following forms are required to be completed and submitted as part of the Confirmation process. Please note the date that each form is due.

Registration (Due June 30, 2011)

• Candidate Questionnaire
• Retreat Permission

Confirmation Name & Sponsor Eligibility (Due December 2, 2011)

Service Reflection Sheet

• Service Reflection Sheets
  (due September 1, 2011 for first 10 hours of service; December 2, 2010 for remaining 5 hours)
  Service must be completed from January 1, 2012 to December 2, 2012. Hours without forms will NOT count toward the 15 hour requirement.
• mail to Mrs. Finley or drop off at her home: 212 Park Drive, Catonsville, MD 21228
• scan and email them to Mrs. Finley at fran@procomgt.com
• drop off at the Parish Center (30 Melvin Ave), St. Mark School (26 Melvin Ave), or Religious Ed class (St Mark School) in an envelope marked “Confirmation Service Hours—Attn: Finley”
• fax them to the Parish Office at 410-747-3182
